Chronic Disease Management Plan
Chronic health conditions are those that last for six months or more, and can require specialised, ongoing, and sometimes complex treatment and care. These may include conditions such as asthma, heart disease, diabetes, arthritis, cancers, chronic pain, or endometriosis. These conditions often affect more than one area of our lives, and the management can feel overwhelming.
A Chronic Disease Management Plan (often referred to as a Care Plan) is available to patients with chronic health conditions, giving access to sessions with a multidisciplinary allied health team to treat the condition from different angles. These may include a range of allied health professionals, such as physiotherapists, dieticians, exercise physiologists, diabetes educators, and many more.
Health Assessments
Health Assessments with Waurn Ponds Medical Centre are available to a range of patients and play an important role in screening for risk factors within a patient’s health, providing your doctor an opportunity for early intervention.
Both Care Plans and Health Assessments are booked for an appointment with both the nurse and the doctor. To ensure these appointments are allocated correctly, please inform our reception team when making your appointment.
