Form Completion

Forms for Licences, Insurance Claims/Applications, Employment Medicals, General Applications and the like, may incur an out-of-pocket and/or non rebated expense.
Please send a copy of the form you wish to be completed to reception@waurnpondsmc.com.au at least 24 hours prior to your appointment time. The GP will assess:

A) if the forms can be completed
B) appointment length (which may require your appointment to be rescheduled to accommodate)
C) approximate cost for the appointment

A Receptionist may be in contact with you regarding the appointment to discuss further, as needed.
Where no forms are received prior to your scheduled appointment, please be advised that the GP may not go ahead with your appointment.

Insurance Claims/TAC/Work Cover

Please ensure that you have a claim number for your incident. Until we receive a letter from your insurer accepting liability for your claim, payment will be the patient’s responsibility.
